HealthLoop operates a SaaS platform that automates the follow-up care by enabling medical practices to monitor signs and symptoms, and communicating with patients during the recovery process. The platform enables doctors to identify patients at risk of decline in the follow up period. It keeps doctors, patients and care-givers connected between visits with clinical information that is "insightful, actionable, and engaging." In addition, the platform collects patients' feedbacks and monitors status against expected progress; and sends alerts to physicians or providers about patients who are at risk of treatment failures, complications, or hospital readmissions. On the other hand, HealthLoop's analytics engine sifts through and filters the deluge of patient-generated data in real-time, focusing the care team's time and attention on patients who need them the most. HealthLoop has reduced readmissions by 30 percent, with 91 percent of patients "Likely to Recommend" it.
